If you are the owner of the Yape account you're trying to withdraw to and the system shows error code 45009, we will need you to send us proof of ownership of your phone line. This document must include:
Full name
Phone number
Logo and/or name of the issuing company
You may submit either a certificate of ownership or a recent phone bill (issued within the last 6 months).
Once you have the document, scroll down to the bottom of this page and open the contact form.
In Category, select KYC, and in Topic, select Payment method verification.
In the Description, briefly explain the reason and add any relevant details (account/username, reference, etc.).
In Attachments, upload the requested document.
